The Circle of Life Award honors programs that:

  • are striving to equitably provide effective, patient-centered, timely, safe, and efficient palliative and end-of-life care
  • are striving to implement the preferred practices for palliative and hospice care quality identified by the National Quality Forum consensus report
  • show innovation and serve as models for the field
  •  support hospitals’ and health organizations’ efforts to improve palliative and end-of-life care
  • build awareness of the importance of serving patients near the end of life well and supporting those close to them
  • are actively working with other health care organizations and the community across the continuum of care

This program will train health professionals in medicine, nursing, physical therapy, psychology, dentistry and social work in the following Institute of Medicine identified priority areas: advanced illness, frailty, care coordination, symptom management, medication management, ethnogeriatrics, and health literacy/self management.
